BRIEF OVERVIEW
The Berkley Manor Flow Diversion Lift Station Rehab Project was awarded by the Board on April 13, 2021, Doc Id. No. 18097, to US Water Services Corporation in the amount of $718,298.86.
Change Order One (1) was for an increase to purchase a larger generator needed to run the new larger pumps and for a time extension to add 208 days needed for lead time to purchase generator. Total increase was for $19,456.05. Purchase order was increased from $718,298.86 to $737,754.91.
Change Order Two (2) was for a no-cost time extension needed to change substantial completion date to June 27, 2022. The extension was due to material and supply demand. Final Completion date remains December 21, 2022.
Change Order Three (3) was for a no-cost time extension needed to change substantial completion date to July 25, 2022. Pump Manufacturer needed to have representative on site for start-up. Final Completion date remained December 21, 2022.
Change Order Four (4) was for an increase due to added material needs for site alignment, increasing the purchase order by $5,455.44 from $737,754.91 to $743,210.35.
Change Order Five (5) was for a no-cost time 90-day extension on Final Completion needed due to continued delays for the generator. New Final Completion date was March 23, 2023.
Change Order Six (6) was for a no-cost 45-day time extension on Final Completion needed due to continued delays for the generator. New Final Completion date is May 5, 2023.
Change Order Seven (7) was for a decrease of funds in the amount of $1,045.07. The project has been completed. This decreases the purchase order by $1,045.07 from $743,210.35 to $742,165.28.
The Final Contract amount is $742,165.28 (PO No. 21000816).
The work has been completed and US Water Services Corporation has submitted its Final Pay Application No. Five (5) in the amount of $131,580.87.
US Water Services Corporation has signed a release of lien. No notices of lien or claims of lien have been filed on the project. All releases properly include Hernando County.
FINANCIAL IMPACT
Funding in the amount of $131,580.87 is available in the following accounts: Account No. 07244-5666587 (CIP-Berkeley Man Flow Div.) in the amount of $99,444.85 and Retainage Account No. 4144-2050218 (Retainage - US Water Services Corporation) in the amount of $32,136.02.

The Board has the authority to take the recommended action on this item pursuant to Chapter 125, Florida Statutes.

RECOMMENDATION
It is recommended the Board approve final pay Request No. Five (5) in the amount of $131,580.87 to US Water Services Corporation, to close out the Berkley Manor Flow Diversion Lift Station Rehab project under Contract No. 21-C00053/DK.
The Final Contract amount is $742,165.28, an increase of $23,866.42 from Board award.
