BRIEF OVERVIEW
The US 19 and Forest Oaks Boulevard Intersection Improvement Project was awarded by the Board on November 16, 2021, Doc. Id. No. 18961, to David Nelson Construction Co. in the amount of $647,678.50.
Change Order No. One (1) was for 50 Linear Feet of replacement curbing, increasing the Contract by $2,000.00 from $647,678.50 to $649,678.50.

RECOMMENDATION
It is recommended the Board approve final pay request No. Five (5) in the amount of $31,425.02 to David Nelson Construction Co., to close out Contract No. 21-C00121/DK for US 19 and Forest Oaks Boulevard Intersection Improvement Project.
The final Contract amount for this unit is $628,500.41, a Contract decrease of $19,178.09 to BOCC award.
