BRIEF OVERVIEW
The Brooksville City Hall Bus Stop Improvement project was awarded by the Board on July 13, 2021, Doc. Id. No. 18475 to Asphalt Millings, Inc., in the amount of $67,719.27.
Change Order No. One (1) was a no-cost time extension due to supply chain delivery issues, extending the completion date 92 days for a final completion date of May 31, 2022.
Change Order No. Two (2) was a no-cost time extension to extend the Purchase Order (PO) to coincide with the Notice to Proceed. The date was extended from May 31, 2022, to August 13, 2022.
The Final Contract amount is $67,719.27 (PO No. 21001085).

RECOMMENDATION
It is recommended the Board approve final pay request No. One (1) in the amount of $67,719.27 to Asphalt Millings, Inc., to close out Contract No. 21-CG0013/DK for Brooksville City Hall Bus Stop Improvement.
The final Contract amount for this project is $67,719.27, a Contract amount equal to BOCC award.
